For the purposes of FORT a referral occurs when one service uses the FORT system to send the details of a particular client to another service or services. Your personal details are shared between agencies signed up to the FORT system. Your details will only be shared between services involved in providing services for your case.

In order to help you we need to maintain and share a record of your case, this may contain sensitive personal data*. The Data Protection Act 1998 says we must get your consent to do this. Everything you tell us will be treated confidentially. For the purposes of the Act the Data Controller is the Fife Rights Forum

Personal Information - Your Right to know

 

Introduction

This document explains how the Fife Online Referral Tracking (FORT) system is complying with the requirements of the Data Protection Act. It also tells you how you can obtain details of personal information we hold about you.

What is the Data Protection Act 1998?

It is designed to protect personal information about living individuals, no matter how it is processed, what it is processed for or who processes it.

 

Can I find out what data the FORT system holds about me?

 

Yes. As a Data Subject you have the right to be:

 

  • Told whether we are processing information about you
  • Provided with a description of the data, why we are processing it and the sorts of people or organisations we might disclose the information to
  • Provided with a copy of the data in an intelligible and permanent form

 

How do I obtain information about me?

 

You must write to the FORT data controller (below), asking to see your records. You will need to provide your name and address, your unique FORT ID (see overleaf) and any other information (e.g. date of birth, proof of address) that could help us find your information.

 

Do I need to provide identification?

 

Yes. To ensure that we only supply information to those who are entitled to receive it, we ask that you provide some means of identification. Acceptable forms are a birth certificate or driving licence. Photocopies are permitted.

 

What will I receive?

 

You will be given a copy of all the information the FORT system holds about you

 

What do I do if the data is incorrect?

 

You must write telling us what data is incorrect and ask for it to be corrected. We must tell you what has been done within 21days of receiving your request.

 

Can I stop you using my information?

 

Yes. You must write to us informing us that you wish to be removed from the FORT system. This will result in the termination of the referral.

 

How long do you hold on to my information?

 

Your information is held on FORT until either the parties involved in your referral agree to close the case or the Data Controller receives a request from you to close the referral. Once the case is closed your details will be archived for 6 years.
